Student Agreement, signed electronically or a copy of the signature, by the Parent/Guardian for a Minor Student.

• I am the Parent/Guardian for the minor student identified below, 18 years of age or older, responsible for all actions by my minor and bound by the Tutoring Online Marketplace, LLC Terms and Conditions (“T&C’s”).
• All information provided during registration was accurate. If any information changes, I agree to promptly notify Tutoring Online Marketplace, LLC.
• I understand that I must have a valid credit or debit card to pay for tutoring sessions of my minor (applies to parent/guardian of a minor student).
• I will ensure that my minor will be on time for each tutoring session, be prepared for each tutoring session and conduct him/herself in an appropriate manner during each tutoring session in words (verbal and written) and conduct.
• I approve of my minor using the Tutoring Online Marketplace (“TOM”) as a tutor/student, without any further consent from me, though I will continue to supervise and monitor his/her use of the TOM (applies to minors at least 14 years old).
• I will supervise all tutoring sessions of my minor (applies to minors under 14 years old).
• I understand TOM does not hire any minors- not as independent contractors and not asemployees.
• Once my minor turns 18, I am no longer responsible for my minor. Upon turning 18, my minor will need to update his/her account. I agree to inform my minor of this process, in order to continue tutoring or receiving tutoring services.

• Accept all responsibility included in this agreement and the T&C’s
• Verify that the minor identified below understands the contractual terms
